CHEMICAL MANAGEMENT UNIT E-FORM
Effective June 2019, all staff/academician that will retire/leaving UTM are compulsory to declare the chemicals they have. Those chemicals must be formally handover to another responsible staff/academician that still in service. This form must be submitted to CMU for the record.
